HOW TO ACCESS YOUR ONLINE CUSTOMER PORTAL
Now that you have been approved for your new plan, you can access your online customer portal which will give you access to your ID cards, policy information, claims, and other useful things that come with your new plan. Simply follow these steps:
- Got to https://myushg.ushealthgroup.com/ and create a new account using the following:
- Policy # 55k5555555 (you can find this in the confirmation email saying you were approved, it’s a 10-digit number)
- Your email address on file
- Click on the confirmation email and enter the temporary password and then setup your account
- Login into your account

HOW TO SEARCH FOR IN-NETWORK PROVIDERS
Another great thing about your private health coverage plan through US Health Group is the network, which is called the United Health Care PPO Network. This is the name that most doctors and hospitals recognize this plan by and is on your ID card.
The great thing about the United Healthcare PPO Network is:
- You can now go to any doctor, any hospital and use the benefits, which means
- You go to the clinics you like and
- You can shop around for health care services, which can vary significantly by provider
- You don’t need a referral to see a specialist
- It’s a nationwide network
You can get an extra network discount when you stay in the network. Because of how large the network is, many doctors and hospitals will be in the network so you can get an extra network discount for health care services. However, to check to see if a doctor/hospital is in-network, just do the following:
- Click on this link: https://connect.werally.com/medicalProvider/root
- Find a provider. (you don't need to sign in or create an account)
- Medical Directory
- Employer or Individual plans
- Choice Plus or Options PPO
- Input the city and state of where you want to search for Doctors/clinics
- Type in the name of the doctor/clinic in the search box to find a specific provider, or scroll down to the links listed under popular searches to do a general search for a doctor, hospital, etc.
